| Course-proposal form, PART A (CPC) | When proposing a course, please read and follow the instructions at the top of the form. Submitted forms will be sent by email to you, the department chair, and the Curricular Policy Committee. CPC will not consider a proposed course until the department chair submits support. |

| Course-proposal form, PART B; Support of Chair (CPC) | Department Chairs, fill out this form in response to a Course Proposal you receive via email. Submitted forms will be sent to the instructor, the department chair and the Curricular Policy Committee. CPC will not consider a proposed course until this has been submitted. |
